Student Performance Report

Please complete this form for each student. Print a copy for your files if desired after submitting this to the Coop Coordinator at Seneca College. Reviewing this information with the student is encouraged but not mandatory.

This form is best completed by the student's immediate supervisor and should serve to aid the employer in discussing the student's work performance, personal characteristics, career plans and the student's suitability to their chosen career.

It will work best if the student and the supervisor discuss work term objectives at the beginning, middle and towards the end of the term.

Evaluation is a continuous process. Periodic feedback will make the student aware of strengths and weaknesses as they become apparent.

If not possible to review the evaluation with the student on-the-job, the Coop Coordinator requests permission to review this evaluation with the student during the return-to-campus interview.
