Leave of Absence Request: Full-Time Program
A student who wishes to take a semester off from a full-time day program is required to complete and submit this form.
- Online: fill in the form using the boxes provided.
- Once completed, print the form and sign it in the space indicated.
- Obtain required signatures from the academic area for approval of the return term.
- International students only: Obtain required signatures from International Student Services.

A student who wishes to withdraw from a full-time day program is required to complete and submit a Withdraw Form: Full-Time Program form.
