Leave of Absence Request: Full-Time Program

A student who wishes to take a semester off from a full-time day program is required to complete and submit this form.

  1. Online: fill in the form using the boxes provided.
  2. Once completed, print the form and sign it in the space indicated.
  3. Obtain required signatures from the academic area for approval of the return term.
  4. International students only: Obtain required signatures from International Student Services.

A student who wishes to withdraw from a full-time day program is required to complete and submit a Withdraw Form: Full-Time Program form.
